肿瘤登记主要收集登记覆盖范围内全部癌症和中枢神经系统良性肿瘤及动态未定或未知肿瘤病例的发病、死亡和生存状态，以及登记覆盖人群的相关人口资料。

个人信息包括姓名、性别、出生日期、年龄、身份证号码、住址、出生地、民族、婚姻状况、职业等；肿瘤信息包括发病日期、解剖学部位（亚部位）、组织学类型、诊断依据、临床分期等；报告单位信息包括报告日期、诊断单位、报告单位、报告医生等。

肿瘤死亡资料来源于全人口死因登记报告，包括根本死因为非肿瘤原因的肿瘤病例的死亡资料。除发病信息外，还应包括死亡日期、实足年龄、死亡原因主要诊断、诊断级别和依据、死亡地点等。

肿瘤病例随访资料包括最后接触时间、生存状态、是否失访、失访原因等。

人口资料来源于我国人口普查资料和公安、统计部门逐年提供的人口资料。人口资料包括居民人口总数及其性别、年龄别人口数或构成。年龄组按0～、1～4、5～9、10～14、…75～79、80～84、85＋分组。为计算肿瘤相对生存率，需提供相应年份的寿命表。

3　Data collection

Cancer registries are required to collect data on all cancers’ incidence，mortality and survival，including tumors of central nerve systems with benign or uncertain behaviors.Thedataof population coverage should also be collected.

3.1　Incidence

We collect personal information of incident cases including age，sex，date of birth，age of diagnosis，identification number，address，place of birth，race，marital status and career.The detailed information on cancer including date of diagnosis，anatomical site and sub-site，pathological，histological as well as cytological results，diagnosis basis and stage are also reported.The reporting date，clinics of diagnosis，reporting bureau，and reporting doctors should be collected.

3.2　Mortality

The cancer mortality data are from population-based all causes of death surveillance database.Besides personal information of cancer incidence，the mortality data should contain date of death，age of death，cause of death，place of death，and diagnostic basis for death cause.

3.3　Survival

The cancer survival statistics are from the follow-up data of the cancer patients.The detailed cancer survival information includes last time of contact，vital status，causes of lost-to-follow up.

3.4　Population data

The population data originate from census data，departments of statistics or public security.The detailed population data should cover the overall population with age-specific data by 5-year age groups and sexspecific data.For relative survival calculation，the cancer registries should also provide life tables.
